- Referring provider's name, practice, callback number, and return fax number.
- Reason for referral, requested services, and urgency.
- Relevant clinical notes, medication list, and insurance information, if available.
Jauregui Psychiatry participates with select commercial insurance plans. Coverage and network eligibility are verified before the patient's first appointment. Referring providers do not need to determine insurance eligibility before submitting a referral.

Adults and adolescents ages 12 and older, located anywhere in Texas, by secure telehealth. Evaluations and ongoing care are available in English or Spanish.

Self-pay rates
Initial evaluation: $250 (60 min) · Follow-up: $125 (30 min). ADHD and bipolar evaluations require a minimum of two 60-minute appointments — a total of $500 for the assessment. Sliding-scale fees may be available based on financial need and availability. Superbills are provided on request for out-of-network reimbursement.
Prescribing limits
Schedule II controlled substances — including stimulant medications and benzodiazepines — are not currently prescribed, due to their extensive side effect and dependence profile. When a patient's presentation calls for one of these medications, an appropriate referral is provided.
Availability
Weekday, evening, and weekend telehealth appointments are offered. Current openings are confirmed by phone, email, or at scheduling — ask our office for the next available time.
Collaborative care
With patient authorization, we welcome collaboration with therapists, psychologists, primary care clinicians, specialists, registered dietitians, and other members of a patient's care team, including diagnostic impressions, medication recommendations, and relevant treatment updates.
How referrals are handled.
Confirm transmission
Your fax system's successful transmission report indicates that the fax was transmitted. It does not confirm that our office has reviewed or accepted the referral.
Referral review and acknowledgment
After reviewing the referral, our office will acknowledge receipt using the referring office's provided fax number or by phone. We will request additional information if needed.
Patient outreach
For referrals appropriate for our services, we will contact the patient or appropriate guardian to discuss next steps, intake paperwork, payment options, and scheduling.
Scheduling update
We will provide the referring office with an appropriate status update, such as whether the patient has scheduled, declined, or could not be reached.
