How your health information is used and protected.

Uses and disclosures of your health information

We may use and disclose your protected health information (PHI) for the following purposes without your separate written authorization, as permitted by HIPAA:

• Treatment. To provide, coordinate, or manage your psychiatric care — for example, discussing your care with a therapist, primary care clinician, or pharmacy involved in your treatment, with your authorization where required.
• Payment. To bill and collect payment for services, including submitting claims to your health insurance plan or to Headway on your behalf, verifying coverage, and processing self-pay transactions.
• Health care operations. For activities such as quality review, billing audits, and the day-to-day business management of the practice.
• As required by law. For example, in response to a valid court order, a public health reporting requirement, or where disclosure is necessary to prevent or lessen a serious and imminent threat to your health or safety or that of others.
• Telehealth and scheduling platforms. Appointments are conducted and booked through secure, encrypted platforms, including CharmHealth and Headway. These platforms process limited PHI — such as your name, contact information, and appointment details — solely to deliver and bill for your care.

For any other use or disclosure not described above or otherwise permitted by law — including most uses of psychotherapy notes, and any marketing or sale of your information — we will ask for your written authorization first. You may revoke that authorization in writing at any time, except to the extent we have already relied on it.

Your rights regarding your health information

You have the right to:

• Request a copy of your medical record and receive it, generally within 30 days of a written request.
• Request a correction to your medical record if you believe it is inaccurate or incomplete.
• Request confidential communications — for example, that we contact you only at a certain phone number or email address.
• Request restrictions on certain uses or disclosures of your PHI. We are not required to agree, except that we must honor a request to restrict disclosure to a health plan for services you have paid for yourself, in full.
• Receive an accounting of disclosures — a list of certain disclosures of your PHI made for reasons other than treatment, payment, or health care operations.
• Receive a paper copy of this notice at any time, even if you agreed to receive it electronically.
• File a complaint if you believe your privacy rights have been violated (see "Complaints" below).

Our duties

We are required by law to maintain the privacy of your PHI, to provide you with this notice describing our legal duties and privacy practices, and to abide by the terms of the notice currently in effect. We will not use or disclose your PHI except as described here, unless you give us written authorization to do otherwise. If we become aware of a breach of unsecured PHI, we will notify you as required by law.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights:

You will not be retaliated against for filing a complaint.

Effective date and changes to this notice

This notice is effective October 6, 2026. We reserve the right to change the terms of this notice and to make a revised notice effective for PHI we already have as well as any PHI we receive in the future. An updated notice will be posted on this page and, on request, provided to you directly.
